I dont know how to simulate birth but try filling a balloon halfway with warm water and the rest of the way with air. This can simulate fetal movement and you can have various sizes for different trimesters

Has anyone ever used a pump up dildo or vibrator to push?

I actually put a balloon inside a balloon. The inner balloon, I fill with warmish water, then tie it off and blow up a little air in the outer balloon and tie it... This way I have some motion, some weight, and even some warmth... I use one of those FoM/Moshi/bead pillows for shape and then put the balloon combo underneath that (closer to my body).... The balloon combo alone just doesn't seem to give quite the right shape for me.

Balloons, pillow, and swimwear/shapewear/whatever you use to keep the belly in place. I use swimwear
