"When you feel a contraction, bear down with your pelvic muscles for ten whole seconds."

One of the monitors beeped. The fetus's heart rate was dropping fast, as well as the mother's blood pressure. They couldn't transfuse blood into her quickly enough to curtail how much she was bleeding from the tears.

Amanda groans in pain having a contraction she grips the bed trying to push down hard

Despite her pushing, the baby barely moved. More blood gushed from her hole.

“Harder!” The doctor ordered.

The first twin progressed exceptionally slowly. Its large shoulders scraped against the tears lining the inside of Amanda's birth canal, sending waves of agony through her body. Her pelvis visibly distended when the fetus entered transition. Teirney worried that Amanda would dislocate the bones if she continued pushing at this rate, but that was a less serious condition than death.

"That's it, Amanda," the doctor urged. She pressed her hands against the woman's hips, supporting the weakest parts of her body. But, of course, it wouldn't be enough to stop the mindblowing pain.

Amanda stops pushing breathing hard then pushes harder again

With the next push, Amanda's small hips popped out of their socket, sending shooting pain through her body. The bones we're likely damaged, even cracked by the strain. Teirney worried the mother would pass out or go to shock but there was nothing she could do to alleviate the pain.

Teirney shouted orders at her aids. They brought another blood transfusion and fluids to try to help Amanda stay conscious. Unfortunately, they couldn't repair her hips while she was in labor and there were no medications she could have for the pain. If she succumbed to shock and passed out, she would surely die and her children too.

"Your children are fine," Teirney assured her, "but you're very injured and you need to birth these children as soon as possible. Keep pushing."

More blood poured out from her birth canal as she entered transition. The baby was tearing the muscles of her tight cervix, causing her unspeakable agony as it moved down.

Teirney put a cold cloth on Amanda's head to help with her fever. "I'm sorry dear, but we don't carry any pain medications that wouldn't poison your child. Elf fetuses are so, so fragile." She truly grieved for her patient's pain and knew if they didn't deliver this child soon, the mother would pass out and die.

As she spoke, the next contraction wracked Amanda's frame, making her shriek in agony. "Push, dear, you need to give your children life."

The baby's head begins to bulge. Pieces of blood and torn bits of tissue begin to spurt out from her birth canal, pushed out by the baby's large head.

“You’re crowning,” the doctor said, “you need to push even harder to get the child out. You’re almost there.”

Amanda's opening began to tear towards her anus as the massive head bludgeoned her vagina. Her dark blood rolled down her thighs and to the bedsheets.
